Apprenticeship Approval

Registered Apprenticeship Program:

Step 1. Review the Program Approval Application Checklist

Step 2. Print and complete the Program Application WSAA Form APP-02

A sample is found HERE.

Attach a copy of the Standards of Apprenticeship with all attachments, including wage schedule, related training program curriculum, and task performance hours to the application.

Step 3. Contact the Wyoming State Approving Agency by email or phone, for VA Form 22-8794 and VA Form 22-1999:

Step 4. Submit the WSAA APP-02, with attachment and VA 22-8794 in accordance with the instructions.

PART II: Veteran Enrollment with the DVA

Step 5. Download or view the Veteran Enrollment checklist.

Step 6. Complete, print, sign, and submit VA Form 22-8864 Training Agreement

Step 7. Download an Apprenticeship Training Record if you do not have a system or form for tracking monthly task hours.
